Strengthening Discharge Planning & Transfer of Care 2016

With an ageing population, more people living with complex health conditions and an overall increase in acute-care admission rates, hospitals are facing a range of challenges in organising the discharge of patients in a safe, timely and personalised manner. This conference will address issues relating to length of stay, iincreasing communication between community health services, improving patient transfer and educating carers and family members to assist in post-hospital recovery, in a way which works towards a national integrated health care approach.
