Root Cause Analysis: Effective Defect Elimination in the Workplace 2013

Effective problem solving is an essential competency in any business today, at individual, team and organizational levels. It is required for both incident evaluation and continuous improvement work projects.

There are three stages in RCA Rt:
1. Focus: so we concentrate fire power and achieve a result
2. Find Causes: so we know why
3. Fix Forever: to stop it happening again

During the workshop we will work through each stage using a combination of:
• Discussion of own examples (syndicates or group discussion).
• Paper exercises, working in groups
• Attacking model problems, working in groups
• Documenting the investigations using the rca2go web based software tool

The workshop will also deal in detail with the problems associated with hidden assumptions and the effective analysis of data and information.
An important part of day two will be used to apply the newly learned skills on real problems from the participant’s own work environment.
